Athlete Clearance Instructions

There are two steps an Athlete must clear for participation in athletic activities

(Including summer programs, Try-outs, practices and games)

1.      Go to AthleticClearance.com . Complete the information and electronically sign all required forms.  Upon completion print the form indicating you have completed the process.  Bring this form to the first day of practice/camp/try-out.  Be aware that you will need emergency contact information and proof of medical insurance to complete the process.  For those who need medical insurance there is a link to purchase school insurance embedded in the program. 
2.      Have a Physical Exam completed and return the completed Physical Examination Form (on the reverse of these instruction) prior to or on the first day of camp/practice.
