All athletes are required to have a valid physical done within the past year, in order to participate in sports. The physical may be done by any physician (MD or DO). If you completed a physical to play a fall or winter sport, you do not need to get a new physical done for spring.
Athletic clearance is a four step process, see below:
Step 1: Visit the athletic clearance website , select CA and log in or register for a new account. Select the year (Ex. 2019-2020), school and sport that the athlete will be participating in. Complete all sections and print the confirmation page. If you have multiple athletes, once you reach the confirmation page simply start a new clearance. You can register multiple athletes under one account. For more detailed instructions, see attachment #1.
Step 2: Have a sports physical completed by a physician. Sports physicals are required annually. The physical and medical history forms are attached, see attachment #2 (physicians are not required to use this form)
Step 3: Print, complete and sign the Henry Mayo "Authorization for Sports Medicine Services and Consent for Treatment" see attachment #2
Step 4: Submit your athletic clearance confirmation page, consent for treatment, medical history form and sports physical to your coach.
The Henry Mayo Newhall Hospital Outreach Athletic Trainer Program shall strive to provide the highest quality of prevention, assessment, treatment, and rehabilitation for all injuries sustained by the student athletes.
The Henry Mayo Newhall Hospital Outreach Athletic Trainer Program delivers athletic training services to the student athletes of the William S. Hart Union High School District. The primary services rendered to the student athletes include: prevention, acute recognition and immediate care of sports related injuries, recognition of when referral to medical practitioner is warranted, and collaboration with healthcare team to ensure optimal rehabilitation, recovery and return to sport for all athletes involved. We strive to provide student athletes of the William S. Hart school district with access to an evidence based healthcare collaboration from acute recognition, immediate management and stabilization, referral by certified athletic training staff to the programs preferred medical providers, access to formal rehabilitation with expert physical therapy staff along with implementation of most current return to play guidelines to ensure optimal return to sports participation with the lowest relative risk for re-injury. In addition, this program will offer educational resources and preventive programs individualized to specific sport to decrease the student athletes’ risk for future injuries.
